Хрупкий (низкоэнергетический) перелом у пациента старше 50 лет<br>является наиболее сильным независимым предиктором последующего перелома, однако<br>доля таких пациентов, у которых после индексного события выполняется денситометрия и<br>инициируется антиостеопоротическая терапия, остаётся низкой во всех системах<br>здравоохранения. Одновременно значительная часть пожилых пациентов ежегодно<br>проходит компьютерную томографию (КТ) органов грудной клетки, брюшной полости и<br>таза по показаниям, не связанным с костной системой. Эти исследования уже содержат<br>количественную информацию о плотности губчатой кости, которая в рутинной практике не<br>используется.<br>Цель. Систематизировать доказательства диагностической точности,<br>прогностической ценности, экономической эффективности и барьеров внедрения<br>оппортунистической оценки минеральной плотности кости (МПК) по данным рутинной КТ<br>у пациентов пожилого возраста применительно к задаче вторичной профилактики хрупких<br>переломов.<br>Материалы и методы. Систематический обзор выполнен в соответствии с<br>рекомендациями PRISMA 2020. Проведён поиск в базах данных PubMed/MEDLINE,<br>Embase, Cochrane Library, Scopus и eLIBRARY.RU за период с января 2010 г. по июнь 2026<br>г. Включались оригинальные исследования диагностической точности, когортные<br>исследования с оценкой инцидентных переломов, анализы экономической эффективности<br>и работы по внедрению, посвящённые измерению рентгеновской плотности губчатой кости<br>позвонков (в единицах Хаунсфилда, HU) или фантом-независимой количественной КТ<br>(QCT) на изображениях, полученных по внекостным показаниям. Both types of fractures are characterized by a high degree<br>of intra-articular involvement, instability, frequent associated soft-tissue injuries,<br>and high rates of complications, including post-traumatic osteoarthritis. Over the<br>past 30 years, the role of computed tomography in the management of these injuries<br>has fundamentally changed, from an adjunctive diagnostic method to a primary tool<br>determining the treatment strategy, surgical approach, fixation method, and timing<br>of surgery.<br>Historically, in the era of conventional radiography, treatment of these<br>fractures was largely based on the surgeon's experience and ability to mentally<br>reconstruct three-dimensional anatomy from two-dimensional images. The<br>probability of errors was high: posterior fragments could be missed, the extent of<br>intra-articular disruption could be underestimated, and the surgical approach could<br>be planned incorrectly. The introduction of CT, particularly three-dimensional<br>reconstruction, has fundamentally changed this situation. Current evidence suggests<br>that CT examination can not only improve diagnostic accuracy but also shift a<br>patient from conservative to operative treatment, influence the choice between single<br>and dual approaches, affect the choice between plate fixation and external fixation,<br>and ultimately improve functional outcomes.</p> Umarov A.A., Rabbimov O.P., Rakhmonov U.U. МРТ, СРОКИ, ЭКОНОМИКА). СИСТЕМАТИЧЕСКИЙ ОБЗОР https://www.boffin.su/index.php/journal/article/view/308 <h1><strong>АННОТАЦИЯ</strong></h1> <p><strong>Обоснование. </strong>Скрытые переломы шейки бедра (на 2–10 % случаев не диагностируются по рентгенограммам) у пожилых пациентов представляют серьёзный клинический вызов. Задержка диагноза приводит к смещению перелома, несращению и некрозу головки бедренной кости. В то же время нет консенсуса относительно оптимального алгоритма обследования: КТ быстрее, МРТ чувствительнее. Экономическая эффективность разных стратегий остаётся неясной.</p> <p><strong>Цель. </strong>Систематизировать доказательства диагностической точности КТ и МРТ при скрытых переломах шейки бедра, проанализировать сроки выполнения исследований, клинические исходы при задержке диагноза, и экономическую целесообразность разных диагностических стратегий.</p> <p><strong>Методы. </strong>Систематический обзор по PRISMA 2020. Поиск в PubMed, Embase, Cochrane, Scopus за 2010–2026 гг. Включались исследования диагностической точности КТ и МРТ, когортные исследования клинических исходов, анализы экономической эффективности. Оценка риска систематической ошибки через QUADAS-2 и CHEERS 2022.</p> <p><strong>Результаты. </strong>Проанализировано свыше 40 публикаций. МРТ продемонстрировала чувствительность 100 % и специфичность &gt;98 %, КТ — 86–100 % и 98–100 % соответственно. Среднее время до диагноза 37 часов. Задержка &gt;48 часов ассоциирована с более высокой частотой осложнений (несращение, некроз, остеоартрит). Алгоритм «КТ первой линии, МРТ при отрицательной КТ и сохранении клинического подозрения» оказался экономически доминирующим, сэкономив ~67 000 евро за период наблюдения. Спектральная КТ с виртуальным отсутствием кальция показывает перспективу, но требует дальнейшей валидации.</p> <p><strong>Заключение. </strong>При скрытом переломе шейки бедра и отрицательной рентгенограмме КТ должна быть методом первой линии благодаря доступности и скорости. МРТ рекомендуется при отрицательной КТ и высокой клинической подозрительности. Интервал от презентации до операции не должен превышать 48 часов. PubMed/MEDLINE ma’lumotlar bazasida chop etilgan sistematik sharhlar, meta-tahlillar, kohort, kesimiy va interventsion tadqiqotlar tahlil qilindi. Asosiy e’tibor O‘SOK bilan og‘rigan bemorlarda periferik endotelial funksiyaning buzilishi, uning kasallik og‘irligi va o‘pka funksiyasi ko‘rsatkichlari bilan aloqasiga qaratildi.</p> <p><strong>Natijalar.</strong> Mavjud dalillar O‘SOK bilan og‘rigan bemorlarda periferik endotelial funksiyaning nazorat guruhlariga nisbatan buzilganligini ko‘rsatadi. FMD bo‘yicha 2023-yilda e’lon qilingan yangilangan sistematik sharhda 34 ta tadqiqot va 1365 nafar O‘SOK bilan og‘rigan bemor hamda 617 nafar nazorat ishtirokchisi tahlil qilingan. Meta-tahlilda O‘SOK guruhida endoteliyga bog‘liq vazodilatatsiyaning o‘rtacha farqi −2,33% (95% ishonch oralig‘i −3,30 dan −1,35 gacha; p&lt;0,001) bo‘lgan. Endoteliyga bog‘liq bo‘lmagan vazodilatatsiya ham pasaygan. Meta-regressiya natijalari yoshning kattaligi, havo oqimi obstruksiyasining og‘irligi va faol chekishning endotelial funksiyaning yomonlashuvi bilan bog‘liqligini ko‘rsatgan. Boshqa sistematik sharh va meta-tahlillarda ham O‘SOKda FMD pasayishi va arterial qattiqlik ko‘rsatkichlarining oshishi qayd etilgan. Endotelial mikropartikullar bo‘yicha tadqiqotlar ularning ayrim subtiplarining O‘SOK, emfizema va kasallik og‘irligi bilan bog‘liqligini ko‘rsatadi. Tizimli yallig‘lanish, gipoksemiya, oksidativ stress va azot oksidi bioavailability pasayishi endotelial disfunksiyaning ehtimoliy o‘zaro bog‘langan mexanizmlari sifatida qaraladi.</p> <p><strong>Xulosa.</strong> Endotelial disfunksiya O‘SOKning faqat o‘pkaga xos bo‘lmagan tizimli komponentlaridan biri bo‘lib, yurak-qon tomir xavfining ortishi va kasallik og‘irligi bilan bog‘liq bo‘lishi mumkin. FMD, arterial qattiqlik va ayrim endotelial biomarkerlar O‘SOKda tomir funksiyasining buzilishini baholash uchun istiqbolli vositalardir. Shu bilan birga, biomarkerlarning klinik diagnostik qiymati va endotelial disfunksiyani nishonga oluvchi davolash strategiyalarining samaradorligi bo‘yicha dalillar hali bir xil emas.
